How Did Sam Die on General Hospital?
General Hospital killed off one of its most popular and long-running heroines, Sam McCall (Kelly Monaco), on October 30, 2024. Here’s a look back at how Sam died, the circumstances surrounding her final moments in Port Charles, and how her death was ultimately revealed to be murder.
How Sam Died on GH
Sam had generously agreed to donate a part of her liver to save the life of Lulu Spencer (Alexa Havins), who had been in a coma since 2020 and whose liver was failing. When she regained consciousness following the surgery, her live-in boyfriend, Dante Falconeri (Dominic Zamprogna), asked her to marry him. She happily accepted and later shared the exciting news of her engagement with her mother, Alexis Davis (Nancy Lee Grahn). Alexis and Sam embraced and expressed their love for one another.
Meanwhile, out in the waiting room, Sam’s former husband, Jason Morgan (Steve Burton), was sitting with her kids, Danny Morgan (Asher Antonyzyn) and Scout Cain (then-Cosette Abinante), praising Sam for her bravery. Suddenly, he heard an alarm sound and watched as Elizabeth Baldwin (Rebecca Herbst) and Portia Robinson (Brook Kerr) raced toward Sam’s room.
A visibly shaken Alexis emerged from the room and met Jason’s gaze. Jason sent the kids off to Bobbie’s and joined Alexis, who explained that something terrible had happened: one moment, she and Sam were happily chatting, and in the next, “Her face just went completely white, and she couldn’t breathe.” Alexis admitted, “I’m scared.”
In Sam’s room, her half brother, Lucas Jones (Van Hansis), joined Liz and Portia in trying to revive the patient. Lucas begged Sam not to quit on him, but eventually, Portia noted that Sam had been in asystole — flatline — for 30 minutes, adding gently, “She’s gone.” Heartbroken, Lucas managed to choke out, “Time of death, 5:48 p.m.”
As the terrible news reached Alexis, Jason and Dante, all three were shattered and paid heartbroken visits to Sam’s bedside to say their final farewell to her. Liz assured Jason that they would get to the bottom of why Sam — who had made it through her surgery with flying colors — had suddenly gone into cardiac arrest.

Who Really Killed Sam on GH?
The doctors were confused as to how Sam could have taken such a sudden turn for the worse, but in the November 20, 2024 episode, Portia revealed that the toxicology report showed that Sam had died due to an overdose of digitalis, a medication she should not have even received in the first place.
The mystery of who administered the fatal dose of digitalis was solved on December 9, 2024 when a flashback showed Cyrus Renault (Jeff Kober) — Lulu’s uncle — sneaking into Sam’s hospital room and injecting the drug into her IV.
Liz and Lucky Spencer (Jonathan Jackson) grew suspicious of Cyrus after learning he’d been secretly visiting Lulu while she was comatose at Turning Woods. Aware that they were closing in on the truth, Cyrus set out to kill Liz. On February 19, 2025, he sneaked into her home and before injecting her with digitalis, he confessed that he had killed Sam because she had interfered with what he saw as God’s plan, as Cyrus believed that receiving a liver transplant would prolong Lulu’s coma and hence her suffering. Cyrus also admitted to killing police officer Dex Heller (Evan Hofer).
Fortunately, Lucky arrived in time to save Liz’s life. As for Cyrus, his own life ended on February 20, 2025, when Josslyn Jacks (Eden McCoy) shot him dead as vengeance for his murder of Dex, who had been her lover.
