Emily took Nikolas' hand. He lamented that he didn't see her when he was in Switzerland, but said maybe it was best so he wouldn't have to explain her to the doctors. Lucky and Lulu arrived, eager to find out of Nikolas got a definitive diagnosis. Nikolas told them there's nothing they can do for him, but noted that his rage and headaches have improved since Emily's funeral. He still speculated that he might have killed Emily. Nikolas asked for Lucky's help in finding out who killed Emily, "even if it was me," Nikolas told his brother. Lulu, Lucky and Emily all insisted Nikolas wasn't guilty. Elizabeth arrived to welcome Nikolas home, and offered her support. Lucky reached out to Elizabeth to clear the air but their conversation was interrupted by a cell phone call from Jason, looking to confirm their meeting. Milo then interrupted their conversation to warn Jason that Johnny was about to make a move. Alexis and Sam arrived, and at Emily's behest, Nikolas leveled with everyone: he has a brain tumor. Sam and everyone encouraged Nikolas that he had a lot to live for.

In New York City, Sonny teased Kate that it will be a lot easier for him to fit into her world than she thinks. Kate's associate Julius mistook Sonny for her next cover model. Their conversation was interrupted by a knock on the door. Sonny answered and it was a thug, who was passing a warning on to Sonny from some of the New York families. Sonny told the thug that Zacchara invaded his turf, he will deal with it and after that things will remain "business as usual." When the day was over Kate suggested that Sonny should realize they live in two different worlds. Sonny insisted he was fine with everything and could handle more. Kate told him to brace himself for the New Year's Eve party they'll be attending tomorrow night.

Trevor advised Johnny on how to seal an upcoming deal that will give them leverage to defeat Sonny and take over Port Charles. Johnny mused that by getting rid of Sonny Trevor has another shot at Kate and a chance to run everything. Trevor denied he had any continued interest in Kate, and coached Johnny on how to handle the negotiation. As Trevor advised Johnny on the meeting, Johnny secretly pulled a gun from the desk and armed himself. Trevor warned Johnny to be firm but not pushy in his meetings. "You're no Anthony Zacchara," Trevor reminded him.

Sam noted that Jason can't even hide his contempt for her any more. She asked why he's so into hating her when he dumped her. Then Sam asked Jason how close things are coming to a war between Sonny and the Zaccharas. When Jason declined to comment Sam tore into him about how she's been injured by past mob actions and insisted she needs to know whether she needs to move her family to safety.

Jerry asked Alexis for legal advice on slander as a way to flirt with Alexis. Alexis accused Jerry of flirting with her so that he could use her and she wouldn't be wise to his illegal activities. Jerry insisted he doesn't operate out of pure greed, and said he's out to protect the people he cares about. After he left, Jax stopped Jerry with a business proposition, and asked them to be partners. Jerry declined Jax's offer, leaving Jax speechless. Jax then told Alexis about how Jerry turned him down. Alexis told Jax she can't tell Jerry what they talked about, but asked Jax if he thinks it's possible his brother has changed.

At Kelly's, Mike offered his condolences to Spinelli about Georgie, and urged him to perk up. Spinelli looked at brochures Georgie had left at Kelly's for her semester abroad, and asked Mike if he could keep one. Then he asked Mike for a job. Mike asked Spinelli if he could look at his blender and see if he could get it fixed. Spinelli overheard Logan confronting Coop about seeing him at the pier the night before. Coop countered by asking Logan what HE was doing down at the docks at that hour.

Johnny went to his meeting and let his temper get the best of him. When he was disrespected Johnny pulled his gun to threaten one of the mobsters, but he was quickly subdued and humiliated in front of everyone. When he was ridiculed by the mobsters Johnny knocked out the guard who took his gun, reclaimed it, and threatened to kill the mobster who dissed him by calling him "Junior." "My name is John Zacchara, and I want to hear you say it," Johnny angrily demanded. Meanwhile, Jerry watched the activity through the window from outside — revealing himself to the audience as the mob kingpin Johnny was planning to make an alliance with.
